Alex Fasolo will join Carlton as a free agent.
The AFL advised late on Wednesday that Carlton had made an offer to Fasolo for his services for 2019 and beyond, as flagged by Triple M's Tom Browne on Tuesday.
Collingwood has been advised they will receive a third-round compensation pick, currently pick 57.
Fasolo played just the one game in 2018 as he battled shoulder and ankle injuries throughout the season.
The 26-year-old has played 101 games, kicking 133 goals.
